10. Accounts
- 10.1 In order to procure Services on this Website and to use certain other parts of the System, you are required to create an Account which will contain certain personal details and Payment Information which may vary based upon your use of the Website as We may not require payment information until you wish to make a purchase. By continuing to use this Website you represent and warrant that:
- 10.1.1 all information you submit is accurate and truthful;
- 10.1.2 you have permission to submit Payment Information where permission may be required; and
- 10.1.3 you will keep this information accurate and up-to-date. Your creation of an Account is further affirmation of your representation and warranty.
- 10.2 It is recommended that you do not share your Account details, particularly your username and password. We accept no liability for any losses or damages incurred as a result of your Account details being shared by you. - 10.3 If you have reason to believe that your Account details have been obtained by another person without consent, you should contact Us immediately to suspend your Account and cancel any unauthorised orders or payments that may be pending. Please be aware that orders or payments can only be cancelled up until provision of Services has commenced. In the event that an unauthorised provision commences prior to your notifying Us of the unauthorised nature of the order or payment then you shall be charged for the period from the commencement of the provision of services until the date you notified us and may be charged for a billing cycle of one month.
- 10.4 When choosing your username you are required to adhere to the terms set out above in Clause 9. Any failure to do so could result in the suspension and/or deletion of your Account.
11. Termination and Cancellation of Accounts
- 11.1 Either Fit For Work or you may terminate your Account. We reserve the right to terminate without giving reasons.
- 11.2 If We terminate your Account, any current or pending orders or payments on your Account will be cancelled and provision of Services will not commence.
12. Services, Pricing and Availability
- 12.1 While every effort has been made to ensure that all general descriptions of Services available from Fit For Work correspond to the actual Services that will be provided to you, We are not responsible for any variations from these descriptions as the exact nature of the Services may vary depending on your individual requirements and circumstances. This does not exclude Our liability for mistakes due to negligence on Our part and refers only to variations of the correct Services, not different Services altogether. Please refer to sub-Clause 13.8 for incorrect Services.
- 12.2 Where appropriate, you may be required to select the required Plan of Services.
- 12.3 We neither represent nor warrant that such Services will be available at all times and cannot necessarily confirm availability until confirming your Order. Availability indications are not provided on the Website.
- 12.4 All pricing information on the Website is correct at the time of going online. We reserve the right to change prices and alter or remove any special offers from time to time and as necessary.
- 12.5 In the event that prices are changed during the period between an order being placed for Services and Us processing that order and taking payment, then the price that was valid at the time of the order shall be used.
13. Orders and Provision of Services
- 13.1 No part of this Website constitutes a contractual offer capable of acceptance. Your order constitutes a contractual offer that We may, at Our sole discretion, accept. Our acceptance is indicated by Us sending to you an order confirmation email. Only once We have sent you an order confirmation email will there be a binding contract between Fit For Work and you.
- 13.2 Order confirmations under sub-Clause 13.1 will be sent to you before the Services begin and shall contain the following information:
- 13.2.1 Confirmation of the Services ordered including full details of the main characteristics of those Services;
- 13.2.2 Fully itemised pricing for the Services ordered including, where appropriate, taxes, delivery and other additional charges;
- 13.2.3 Relevant times and dates for the provision of the Services;
- 13.2.4 User credentials and relevant information for accessing those services.
- 13.3 If We, for any reason, do not accept your order, no payment shall be taken under normal circumstances. In any event, any sums paid by you in relation to that order will be refunded within 14 calendar days.
- 13.4 Payment for the Services shall be taken via your chosen payment method, immediately for any setup fee that corresponds to the service plan you purchased and at the same day of each subsequent month (“billing cycle”) for charges accrued during the previous month (“billing cycle”) AND/OR as indicated in the order confirmation you received.
- 13.5 We aim to fulfill your Order within 2-3 working days or if not, within a reasonable period following your Order, unless there are exceptional circumstances. If we cannot fulfill your Order within a reasonable period, we will inform you at the time you place the Order. Time is not of the essence of the Contract, which means we will aim to fulfill your Order within any agreed timescales but this is not an essential term of the Contract and we will not be liable to you if we do not do so. If the Services are to begin within 14 calendar days of Our acceptance of your order, at your express request, you will be required to expressly acknowledge that your statutory cancellation rights, detailed below in Clause 14, will be affected.
- 13.6 Fit For Work shall use all Our reasonable endeavours to provide the Services with reasonable skill and care, commensurate with best trade practice.
- 13.7 In the event that Services are provided that are not in conformity with your order and thus incorrect, you should contact Us immediately to inform Us of the mistake. We will ensure that any necessary corrections are made within five (5) working days.
- Additional terms and conditions may apply to the provision of certain Services. You will be asked to read and confirm your acceptance of any such terms and conditions when completing your Order.
- 13.8 Fit For Work provides technical support via our email. Fit For Work makes every effort possible to respond in a timely manner but we do not guarantee a particular response time.
14. Cancellation of Orders and Services
We want you to be completely satisfied with the Products or Services you order from Fit For Work. If you need to speak to us about your Order, then please contact customer care by email at ffwinstitute@wellworkforce.com. You may cancel an Order that we have accepted or cancel the Contract. If any Specific Terms accompanying the Service contain terms about cancelling the Service, the cancellation policy in the Specific Terms will apply.
- 14.2 As specified in sub-Clause 13.6, if the Services are to begin within the cooling off period you are required to make an express request to that effect. By requesting that the Services begin within the 14 calendar day cooling off period you acknowledge and agree to the following:
- 14.2.1 If the Services are fully performed within the 14 calendar day cooling off period, you will lose your right to cancel after the Services are complete.
- 14.2.2 If you cancel the Services after provision has begun but is not yet complete you will still be required to pay for the Services supplied up until the point at which you inform Us that you wish to cancel. The amount due shall be calculated in proportion to the full price of the Services and the actual Services already provided. Any sums that have already been paid for the Services shall be refunded subject to deductions calculated in accordance with the foregoing. Refunds, where applicable, will be issued within 5 working days and in any event no later than 14 calendar days after you inform Us that you wish to cancel.
- 14.3 Cancellation of Services after the 14 calendar day cooling off period has elapsed shall be subject to the specific terms governing those Services and may be subject to a minimum contract duration.
15. Privacy

- 16.2.3 Inform you of new products and services available from Us. You may request that We stop sending you this information at any time.

If We are required to make any changes to these Terms and Conditions, these changes will apply automatically to any orders currently pending in addition to any orders placed by you in the future.

Fit For Work uses industry best practices to provide a high uptime, including a fault-tolerant architecture hosted in cloud servers. We give no warranty that the Website or Facilities will be free of defects and / or faults and we do not provide any kind of refund for outages. We provide no warranties (express or implied) of fitness for a particular purpose, accuracy of information, compatibility and satisfactory quality.
- 19.2 We accept no liability for any disruption or non-availability of the Website resulting from external causes including, but not limited to, ISP equipment failure, host equipment failure, communications network failure, power failure, natural events, acts of war or legal restrictions and censorship.
20. Limitation of Liability
- 20.1 To the maximum extent permitted by law, We accept no liability for any direct or indirect loss or damage, foreseeable or otherwise, including any indirect, consequential, special or exemplary damages arising from the use of the Website or any information contained therein. You should be aware that you use the Website and its Content at your own risk.

21. Anti-Discrimination Policy

We will not tolerate harassment in any form –verbal, non-verbal, physical or visual --towards others based on characteristics or cultures that may differ from our own.
- 21.2 Conduct that creates an environment of hostility, threat, or coercion is prohibited. Degrading or humiliating jokes, slurs, intimidation or other harassing conduct is not acceptable.
- 21.3 We are equally committed to a violence free work environment, and we will not tolerate any level of violence or the threat of violence in the workplace.
- 21.4 There will be no reprisals against any employee who report problems.
- 21.5 While this policy pertains primarily to employees and agents, it protects patients, clients, visitors and vendors as well.
22. No Waiver
In the event that any party to these Terms and Conditions fails to exercise any right or remedy contained herein, this shall not be construed as a waiver of that right or remedy.
23. Previous Terms and Conditions
In the event of any conflict between these Terms and Conditions and any prior versions thereof, the provisions of these Terms and Conditions shall prevail unless it is expressly stated otherwise.
24. Third Party Rights
Nothing in these Terms and Conditions shall confer any rights upon any third party. The agreement created by these Terms and Conditions is between you and Fit For Work.
25. Communications
- 25.1 All notices / communications shall be given to Us either by mail to Our Premises (see address above) or by email to ffwinstitute@wellworkforce.com. Such notice will be deemed received 3 days after posting if sent by first class post, the day of sending if the email is received in full on a business day and on the next business day if the email is sent on a weekend or public holiday.
- 25.2 We may from time to time, if you opt to receive it, send you information about Our products and/or services. If you do not wish to receive such information, please click on the ‘Unsubscribe’ link in any email which you receive from Us.

Dr. James Rethaber, Ph.D., CPE, LSSMBB, Vice President – Technical Operations at Fit For Work, provides strategic direction for the Safety, Ergonomics, and Testing departments/initiatives, as well as process mapping and streamlining of processes to increase operational efficiency/effectiveness for internal and external clients. Dr. Rethaber also oversees the onboarding and continuing education initiatives to assist Fit For Work staff with ever-changing/evolving needs of our internal and external clients.
Prior to joining Fit For Work in 2007, Dr. Rethaber worked in the education and healthcare fields. Dr. Rethaber has over 20 years of experience in the fields of engineering, psychology, and musculoskeletal injury prevention, and is a published author/researcher and presenter/speaker at state, regional, and national conferences.
Dr. Rethaber has a Bachelor of Science in Sports Medicine, a Master of Science in Exercise Physiology, a Master of Science in Industrial Engineering, a Master of Science in Applied Psychology, and a Ph.D. in Human Performance Technology (Training & Development). Dr. Rethaber is a Board-Certified Professional Ergonomist (CPE) and is a Lean Six Sigma Master Black Belt. Dr. Rethaber serves on industry advisory boards at Texas A&M University – College Station and Texas State University – San Marcos and is a former member of the Board of Directors with the Board of Certification in Professional Ergonomics.
Bethany Gardner
Bethany Gardner has been an occupational therapist since 2008 working in occupational health settings in a variety of roles including as a clinician, consultant, academic researcher, and ergonomics engineer. She has worked in various industries including manufacturing, construction, and healthcare. Bethany began working for Fit For Work in 2017 as an Area Manager overseeing a large-scale injury prevention program including onsite early intervention, ergonomics, and employment testing services. She is currently Vice President - Service Development where she manages several company initiatives geared toward operational and program excellence and company growth. Her experience as a clinician and testing provider, manager of employment testing providers and programs make her well qualified to provide high-quality continuing education instruction.
John Derington
John is currently serving as the Continuing Education Coordinator at Fit For Work and resides in Layton Utah. He has helped Fit For Work become approved providers of continuing education units for several healthcare professions. He received his Masters in Athletic Training from Brigham Young University in 2014. He has maintained his certification and licensure in athletic training since 2011. John worked for The University of Kansas Medical System for 5 years as an athletic trainer covering various professional, college and high school events. He has also been working as an injury prevention specialist for Fit For Work since 2019, serving clients in both the manufacturing and mining industries.
Lauren Brizendine
Lauren is the Director of Ergonomics at Fit For Work. She provides ergonomic consulting services for clients in the industrial and office settings. She has a BS in Animal Science and a Master of Public Health (MPH) in Environmental and Occupational Health from Texas A&M University. Lauren is a Board-Certified Professional Ergonomist (CPE).
Lauren has been with Fit For Work since 2013 and has worked with clients in nearly every industry across the United States while completing over 7,000 physical demands analyses, ergonomic risk assessments, functional job descriptions, and the development of physical ability testing.
Lauren also serves on the industry advisory board at the Ergonomics Center at Texas A&M University - College Station, TX.

Rachel Beck
Rachel joined Fit For Work in September of 2018 as an Injury Prevention Specialist in Salt Lake City and now also serves as an Area Manager. She earned an undergraduate degree in Biology from BYU-Idaho and taught Science and Health for a time, prior to going back to school to earn a Masters degree from Texas Tech University Health Sciences Center in Athletic Training. While at Texas Tech University she was able to work with high profile collegiate athletes with Track & Field as well as Football. Some of those Track & Field athletes have since gone to the Olympics.
Originally Rachel grew up in Western Washington and enjoys spending as much time as she can outdoors. She considers herself extremely lucky to live in Utah where the outdoor community is quite large. Cycling/mountain biking, gardening, cooking in her Dutch oven for friends and family, DIY projects, and reading a good book are a few of her favorite things.
Wendy Chelette
Wendy Chelette has been an occupational therapist since 1989, working in a variety of clinical settings before shifting her focus to occupational health and injury prevention in the late 1990's. Wendy began her career with Fit For Work in 2005, providing a variety of services including onsite early intervention, ergonomics consulting, and post-offer employment testing services for a variety of employers within warehousing, manufacturing, and construction industries and large call centers. In her current role as Director of Employment Testing for Fit For Work, Wendy supports multiple teams of employment testing providers serving large US employers in the provision of employment testing services.
Kyle Braulick
Kyle attended North Dakota State University and Minnesota State Mankato. Kyle joined the FFW family in September 2012. Kyle has been an Certified Athletic Trainer since 2010. Since that time I have strived to gain as much experience and knowledge to continue to grow as a professional. I seek out educational opportunities and aim to gain more certifications to further my career in Athletic Training. My goal is to provide the gold standard of care to patients based off the most recent evidence based medicine practices. He serving as an Regional Director in Minnesota.
Kyle and his wife Jessica have 2 boys, Jackson. and Kameron. Kyle enjoys golfing, outdoor activities, sporting events, reading, and DIY projects around their home.
Mark Snow
Mark Snow has been working as a Injury Prevention Specialist for Fit for Work since 2021. Previously, Mark was the Dean of the School of Human Performance and Program Director for the Master of Athletic Training Program at Midland University in Fremont, Nebraska. Mark received his Doctorate of Education Degree in Kinesiology from the University of North Carolina Greensboro in 2021.
Mark has been working as a Certified Athletic Trainer (ATC) since 1996. In his 25+ years of experience he has worked in the professional, collegiate, high school, clinical, educational, and industrial settings. His years of experience and expertise in leadership, athletic training, and education make him well equipped to provide quality instruction as a continuing education faculty member.
Curtis Kopotic
Curtis Kopotic, MS, ATC, ATR, serves as the Account Experience and Relations Manager to UNFI, along with providing some onsite coverage at the Hopkins Minnesota UNFI distribution center for Fit for Work. He is an athletic trainer, who earned his bachelor’s degree from Brigham Young University in Athletic Training, and his master’s degree from the University of South Carolina in Physical Education with an emphasis in Athletic Training. Along with being the lead host of Workplace Injury Prevention, a Fit For Work Podcast, he also was the editor for both he podcast as well as video content creating.
Sean Pilola
Sean Pilola is an Ergonomic Specialist. He obtained a bachelors degree from Texas State in 2013 and a masters in athletic training from the University of Houston in 2019. He started with FFW as an on-site Injury Prevention Specialist in Houston, Texas working directly with HEB grocery employees. In 2020, he moved to the ergonomic department and has spent the last 3 years traveling North America completing ergonomic consulting projects. His hobbies outside of work include CrossFit, coaching Crossfit, trail riding, and any outdoor adventures with my dog, Bo.
Anna Gibilisco
Anna Gibilisco is the Learning and Development Coordinator servicing Fit For Work out of Omaha, Nebraska. She graduated from the University of Nebraska with a Bachelor of Science in athletic training where she had the opportunity to work with the Nebraska Cornhusker football team for two years. She completed her Master of Science in Health Services Administration with a concentration in institutional leadership at the University of Wyoming. While at the University of Wyoming, she worked with the track and field team and football team.
She loves football (especially the Huskers), reading books, and drinking tea. A goal that Anna is currently working on is to become fluent in Italian while she works on her dual citizenship through her Italian lineage. Anna loves being a part of the Fit For Work family and the opportunity to help employees new and veteran grow!
Kristen Sims
Kristen is a San Antonio native, attending Texas Lutheran University for her undergraduate education. She completed a Bachelor of Science in Athletic Training and a Bachelor of Science in Kinesiology in 2016. She then continued to Terre Haute, Indiana to complete her Doctorate in Athletic Training at Indiana State University. Her Doctoral work was heavily focused on Adult Learning and Simulated Learning Strategies. Kristen then completed just over three years of work with a large Health System in East Texas, serving as an Orthopedic Technician and Education Director. In the fall of 2022, Kristen returned to her hometown of San Antonio to begin a career with Fit For Work as an Onsite Provider. Since joining, she has been a part of many projects and initiatives from onsite work to educational endeavors.
Jacob Wilkins
Jacob Wilkins is an Ergonomics Specialist for Fit For Work in San Antonio, TX. He provides ergonomics consulting services for various clients in the industrial and office settings. Jacob has a BS in Kinesiology from Bowling Green State University.
He has spent 16 (+) years in the injury prevention/ergonomic field. Jacob’s career began in 2006 with a truck manufacturing facility in San Antonio. He joined Fit for Work in March of 2016. His work experience has allowed him to work with employees of all crafts, such as warehouse order selectors, utility workers, manufacturing laborers, and office personnel. Jacob has performed hundreds of physical demands analyses, ergonomic risk assessments, and onsite injury prevention trainings.
Jacob currently resides in San Antonio, TX with his wife Delayna and five children. They are heavily involved with the foster care community, as well as within their church family. For fun, Jacob enjoys spending time outdoors with his family services.
Jarred Binner
Jarred Binner is originally from Pennsylvania but currently resides in Mississippi with his fiancé and two cats. While not at work, you’ll find him on his Harley-Davidson and/or at a classic car show. Jarred graduated from California University of Pennsylvania with his Bachelor’s of science in Athletic Training and Master’s of Science in Rehabilitation. His past experience includes working as a Head Athletic Trainer for the Lancaster Barnstormers in the Atlantic League of Professional Baseball and the Assistant Athletic Director for Sports Medicine at Jackson State University. Currently at Fit For Work, he is an Injury Prevention Specialist for an asphalt and gravel client, as well an Ergonomics Specialist, a Fit For Work Mentor, a Velocity EHS ambassador, and a Fit For Work Institute Early Intervention Roundtable Facilitator.
Jessie Jackson
I have been an ATC for 23 years and have worked in a variety of settings as an ATC and also a teacher for high school and college students , and with the military. I have also worked as a health coach which I found interesting and rewarding. I have been with FFW for nearly 2 years as an Injury Prevention Specialist and more recently an Ergonomic Specialist. In my free time, I am with my 17 year old daughter or at the gym, lifting as training for my next powerlifting meet.
Jerroed Roberson
Dr. Jerroed Roberson PT, DPT is an onsite testing services provider with Fit For Work, a company that specializes in industrial/occupational health and safety. Dr. Roberson joined Fit For Work as a PRN employee in June of 2021 and shortly after, he moved to full time employment as the lead testing therapist for an onsite testing location in Temple, Tx.
Originally, Dr. Roberson is from the greater Washington D.C. area, but he relocated to Texas in 2006. Shortly after his arrival in Texas, he completed his first undergraduate degree at University of Texas at Arlington (UTA), graduating with honors with a Bachelor of Arts Degree in History. A few years passed by and Jerroed decided to refocus his energy on a new career. He re-enrolled at UTA, and during his studies he worked on numerous research projects resulting in multiple recognitions and awards. In 2013 graduated with honors with a Bachelor of Science Degree in Kinesiology with a clinical focus. From there – Jerroed went on to earn his Doctorate of Physical Therapy from Texas State University in 2017.
In 2020, he moved from outpatient physical therapy into occupational health and injury prevention where he now finds much more enjoyment and satisfaction with his work.
Outside of work, you will almost certainly find Jerroed pushing the limits with mountain biking, playing recreational ice hockey, caring for his cactus/succulent collection, and enjoying time with his friends and family.
Maria Lotan
Maria graduated from Michigan State University in 2016, earning a bachelor’s in both Athletic Training and Kinesiology. After school, she moved out to North Carolina with her husband and worked in the clinic setting for 2 years. During this time, she also provided outreach services for the local youth soccer league and provided PRN coverage at local high schools. In 2018, Maria and her husband decided to live closer to family and moved back to their home state of Michigan. Maria joined Fit For Work in June of 2018 and has been working with the company ever since. Currently, she provides services as an Onsite Injury Prevention Specialist and Ergonomic Specialist across clients in Michigan. She enjoys continued learning and along with her focus in ergonomics, has begun taking safety courses to further expand her knowledge.
Jason Heinold
Jason joined FFW in June 2017. He graduated from Eastern Illinois University, where he earned a BS in Physical Education with an option in Athletic Training. He is a certified Athletic Trainer as well as a certified Performance Enhancement Specialist through the National Academy of Sports Medicine. He previously worked for a regional hospital in Urbana, IL for 17 years where he managed the performance enhancement programs for athletes, fabricated custom orthotics and did wellness coaching for a manufacturing plant. Jason is an Injury Prevention Specialist, Ergonomics Specialist and Safety Specialist for FFW.
Jason is originally from Mahomet, IL and recently relocated to Christiansburg, VA where he and his wife plan to spend more time outdoors with their 2 dogs. He and his wife love to travel the world and can’t wait to plan their next adventure.

Forrest Richardson
Forrest is the FFW Safety Division Director providing OSHA, DOT, and EPA compliance services. Forrest also directs the Safety Subscription Program across the U.S. Forrest has over 27 years of experience in occupational health & safety management. He is a board certified Safety Professional (CSP) and board certified Associate Risk Management Enterprise (ARME). Forrest is an authorized OSHA outreach instructor for both OSHA general and construction industries. Forrest serves as the current ASSP South Texas chapter awards and honors chair. He has been a guest speaker for local, regional, and national organizations facilitating hundreds of sessions across many different industries.
Forrest proudly served in the United States Army for 10 years. It was during his military career that he entered safety as a specialty, initially responsible for all safety related maintenance operations for the 1st Cavalry 615th Aviation Support Battalion providing Battalion level safety engineering inspections and training and later becoming lead Army Material Readiness Survey (ARMS) auditor for military bases. Forrest earned a Bachelor of Science Degree in Business Management Science.
Forrest was born in San Antonio, Texas. He and his wife Shelley have two grown sons, Josh, and Joel. Their family has been blessed by a grandson, Noah. Forrest and Shelley have their own rental property business B.P. Dale & Associates. Forrest and Shelley enjoy real estate investment, motorcycles, hiking and golf.
Matt Weber
Matt is a Louisville, KY native who graduated undergrad at the University of Kentucky and then received his MBA from Mississippi State University. He has been with Fit For Work since the beginning of 2016 and is currently the VP of Strategic Accounts managing the Business Development team.
Matt lives in Austin with his husband, Ryan, and son, Ethan who was born at the end of 2021. In his free time, he enjoys being very active in sports and socializing, trying new restaurants (he hasn’t met a queso he doesn’t like), and traveling.
Monica Steinborn
I am Monica Steinborn. I have been an Athletic Trainer for 20+ years in which the majority of this time was in a clinic and on the sidelines. I transitioned to the industrial side of the field and have been with FFW for 5.5 years. I provide onsite services at various sites in West Central Iowa. I am an Ergo Specialist and Safety Specialist.
I like to spend time with my husband and 3 grown sons. Our 3 furbabies are also joining in our fun. My main daily activity is walking 2 of our rescues Phoenix and Khosa. Of note, Khosa was adopted from Romania through a rescue.
